SHM Be Well Agreement

We are excited to have you join us in person at the SHM New Hampshire/Vermont Chapter Winter Conference.  Our attendee's comfort and safety are of the utmost importance to us, therefore SHM is requiring all those in attendance to be fully vaccinated.  SHM reserves the right to verify vaccination status. Any public space where other people are present holds an inherent risk of exposure to COVID-19 and other communicable diseases. By attending this event, you agree to voluntarily assume all risks related to exposure and agree to not hold SHM or any of their affiliates, including partners and sponsors, directors, officers, employees, agents, contractors, volunteers, or sponsored venues liable for illness.  You will take necessary precautions while at this event including, but not limited to, engaging in appropriate social distancing, wearing a mask in public areas when not consuming food or beverages, minimizing face touching, frequently washing hands, and avoiding risky environments such as overcrowded bars or restaurants. You agree to not attend any SHM event if you feel or had recent exposure to a COVID-19 case.
